• Conduct Interviews, assesses, and evaluates the needs of clients: • Assesses the psychological, social, psychosocial, and physical needs of clients • Conducts home visits; evaluates housing and stability of clients; obtains release authorizations for and collects demographic, medical, criminal, mental health, substance abuse, abuse and neglect, military, employment, education and financial information, records, and reports; and determines medication compliance. • Coordinate resources, referrals, and services to meet the needs of clients • Determine appropriate resources, referrals, and placement options based on client needs; counsels clients; • Shares client information with courts, agencies and community provides as legal and appropriate; and coordinates client placements. • Compiles data for further processing or for use in preparation of department reports; and maintains computerized and/or hardcopy records. • Operates and maintains a motor vehicle; operates a personal computer to enter, retrieve, review or modify data, utilizing word processing, spreadsheet, database, presentation, Internet, e-mail, or other software; and operates general office or other equipment as necessary to complete essential functions.

• Helping group home residents modify their behaviors to reflect on issues to help them make positive changes • Interviews residents to obtain information concerning social history, treatment and other pertinent information. • Plans and administers therapeutic treatment, behavior modification, and stress management therapy to assist residents in developing/displaying appropriate behaviors. • Observes residents to detect indications of abnormal physical or mental behavior. • Reviews results of tests given by psychologists to evaluate residents’ needs. • Changes method and degree of therapy when indicated by resident reactions/needs. • Discusses progress toward goals with residents such as medication regiment, family relationships, educational programs, social development, discharge plans or other behavioral problems. • Consults with psychologist/psychiatrist or other specialists concerning treatment plans and amends plans as required. • Maintains regular contacts with other agencies/professionals regarding client progress in the therapeutic treatment program through phone calls, letters, and/or monthly reporting. • Refers resident to supportive services to supplement treatment and counseling.

• Advocates for client in court: presents treatment plans and mitigating information to courts; testifies regarding the client's competency, stability, and ability to be placed in treatment programs; and educates and advises court officials regarding mental health, substance abuse, indecency issues, placement options, and related concerns. • Reviews client's progress and compliance with programs: monitors attendance; encourages participation and achievement of objectives; • Consults with jail staff, mental health professionals, courts, pharmacies, and others to ensure stability, safety, and compliance of clients; and performs related case management processes and functions. • Conducts outreach activities to develop community partnerships and social service resources: • Collaborates and networks within the social service community; and conducts phone interview and field visit of placement facilities.

• Identifies patient care requirements by establishing personal rapport with potential and actual patients and other persons in a position to understand care requirements. • Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families. • Promotes patient's independence by establishing patient care goals; teaching patient, friends, and family to understand condition, medications, and self-care skills; answering questions. • Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following hospital and nursing division's philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations. • Resolves patient problems and needs by utilizing multidisciplinary team strategies. • Maintains safe and clean working environment by complying with procedures, rules, and regulations; calling for assistance from health care support personnel. • Protects patients and employees by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations.
