Admissions coordinator and health information manager

Dec 2017 - Apr 2018
Westchester senior living
Took referrals from the community and hospitals for admission into our facility for various services ranging from skilled, respite stays or long-term care. I would get authorization from insurance prior to admission and relay diagnosis reasons for admission. I spoke to case workers from hospice, insurance, government or hospitals. As health information manager I was responsible for opening charts for new admits, taking their pictures, preparing their rooms, updating medical records, data entry, performing audits, closing charts, responding to medical record requests and obtaining patient/poa consent.

CMA

Feb 2017 - Nov 2017
Catholic Charities
I worked with clients with IDD in a day program setting. I would work together with my clients to create a monthly calendar of activities and volunteer work we would be participating in their attendance. I acquired my CDL passenger licence in order to drive a company bus to take my clients into the community for lunch once a month at a restaurant of their choice, drive them to the YMCA for swimming and playing sports, I would also coordinate volunteer projects with local businesses that would allow my clients to gain job skills and also educate our town about the IDD community within our population. I also assisted clients with ADLs and administered their medications. It was by far the greatest opportunity I have had the pleaaure of experiencing but unfortunately I had to leave my clients when I moved to Arizona to be close to my family.
