Provided case management services to Mendocino County Mental Health clients who are currently placed within twentyfour hour care. Additionally responsible for the clinical assessment of prospective clients into multilevel system of care. Highlighted Contributions: Conducted routine assessments for adequate services and appropriate level of care placement for a caseload of 20+ clients. Active member of the multidisciplinary team by providing detailed mental status reports weekly to the Chief Medical Officer, Clinical Director, Access Officer, Director of Care Management Services, and Director of Utilization Review. Responsible for discharge planning of patients returning to the community from psychiatric inpatient stabilization. This includes coordination of transportation, outpatient case management services, scheduling of psychiatric followup, medical followup, substance abuse treatment followup, and any additional services recommended by discharging physician. Responsible for overseeing the admission process for prospective clients in 9 different facilities which include, Mental Health Rehabilitation Center, Secured Intensive Residential Care Center, Structured Adult Residential Care, and 4 Community Board and Care Centers.

Responsible for providing supervision of the centrally stored medication administration program. Additional duties included, maintaining medications for 60 clients, medication administration, provide medical supervision to clients for any health or medication related issues. Highlighted Contributions: Facilitated and instructed all staff medication education through the form of 16 hour course training. Monitored and supervised medication staff to ensure safe preparation, storage, administration, and documentation were being completed as per state licensing regulatory laws. Coordinated clients medical care by scheduling appointments and communicating directly with the appropriate physician for emergency and nonemergency situations. Responsible for medication refills, prior authorization/TAR requests ensuring that all medications are in stock as ordered per the physician. Responsible for processing any new order (written, verbal, or telephone) given by physician's. Monitored all current health conditions for 60 clients, reporting directly to the physician of any adverse reactions, side effects, delirium, or any sudden change in clt's medical condition.
