Having been a detailed, patient and loving caregiver for my mother during the last four years of her life has opened my eyes to the needs of so many recipients like her. Just being there for someone who suffers from Dementia can mean so much to them and their caregivers. A personal achievement that I am proud of is the creation of a Personal Medical Log. This is a medical history record sheet that includes a listing of current illnesses, medications, their doses and purposes and the prescribing doctors. This sheet also states medications that cause allergic reactions and what those reactions are, immunization records and dates , prior surgeries, a listing of doctors, their telephone and fax numbers, their type of practice and family contacts with their names, telephone numbers and relationship to the patient. This PML is updated whenever any change is made in medications, doctors,etc. A copy is given at every appointment, or UC/ ER visits. Copies were given to my sisters in the event that they had to take our Mother to an appointment or were caught in an ER situation. This has been a great time and stress saver!

As a Provider for my mother under the IHSS program, I was responsible for her physical care and home environment. This included: scheduling and accompanying her to all doctors and dialysis appointments, ordering, organizing and dispensing oral and injected medications, and the daily to three times a day checking of both blood pressure and blood glucose levels. I also did grocery shopping, cooked three meals each day, and one bedtime snack. I did light housekeeping, dishes, laundry and ironing.

As librarian for St. Joseph Elementary School, I was responsible for the ordering, weeding and up-keep of all library materials, creating and applying weekly lesson plans, which included a monthly theme to students in grades Pre-K through eighth. Each session included the return and check out of books, and a grade level library lesson. I was also the computer instructor for students in grades Pre-K through fourth, the art instructor for fifth grade and the substitute teacher for grades kindergarten through eighth. I began working at St. Joseph Elementary School in 1995 at the After Care aide and was promoted the following year to AC instructor. I was also the aide for kindergarten and the school office for a time.
