* Provide daily rounds on woundcare patients * Properly cleanse and dress the wounds according to wound physician orders (wound vacs, dry dressings) * Proficient In PCC/Wound Rounds * Weekly wound assessments and measurements * Weekly rounds with wound physician * Monitor for any signs of infection * Provide care to our patients as needed * Measuring and cleaning wounds, proper documentation of wound drainage and characteristics * Following facility protocols for fall risk, transferring and isolation * Stocking and maintaining a safe/clean environment Assist on the subacute/skilled Nursing units when needed.

Assist with the daily operation of patient care, vitals, placing patients in rooms, documenting charts, phlebotomy * Measuring and cleaning wounds, proper documentation of wound drainage and characteristics * Following facility protocols for fall risk, transferring and Isolation * Proficient with EMR (E-Clinical, Quest 360, Epic) * Stocking and maintaining a safe/clean environment * Proper procedure for cleaning and transporting instruments * Wrapping wounds as ordered by Case Manager and Provider, dressing changes (wound vacs, cast removals, two layer wraps) * Perform Task provided by RN and provider

* Makes frequent rounds to note resident/patient conditions and to ensure nursing personnel are performing their work assignments in accordance with acceptable nursing standards. * Gives/receives the nursing report upon reporting in and ending shift duty hours. * Ensures assigned work area (i.e. nurse station, med. Cart, medication room, etc.), Resident/patient care rooms and treatment areas are maintained in a clean and sanitary manner. * Monitors supplies and orders as needed. * Directs and supervises the day-to-day certified nursing assistant services for assigned unit to assure that care is being rendered in accordance with current federal, state and local standards, guidelines and regulations. * Responsible for resident/patient care * Provides required patient assessments/observations * Reviews care plans, at least daily, to ensure appropriate care. * Takes and records blood pressure and vital signs. * Administers catheter care as required (may include catheter insertion). * Provides respiratory care, e.g. oxygen, nebulizer treatments. * Assists the physician in therapeutic measures, as needed. * Receives and transcribes physician's orders to resident's/patient's charts, Kardex/Care * Medication Administration Record ("MAR"), treatment/care plans, as required. * Prepares and administers medications and performs treatments as ordered by the Physician.
