Follow care plan for patients, report immediately to case manager anything unusual about patients. Observe physical and mental condition, intake and output. Give shower, tub bath, or bed bath, inspect feet and skin, apply lotion and protective cream, give oral and/or denture care. Assist with feeding, offer or encourage fluid. Active/passive range of motion Make occupied bed. Help with toileting, give perineal care to incontinent patients. Care and cleaning of indwelling catheter, empty colostomy bag. Transfer from bed to chair or chair to bed ensuring proper body mechanics. Use of mechanical lift when necessary paying close attention to safety of the patient. Universal precaution and infection control, washed my hands frequently. Give care to patients with various medical conditions including Parkinson's, Diabetes, Cancer, and Dementia/ Alzheimer's.
