· Diligently adheres to company procedures and policies. · Over 1-year experience caring for residents in their personal homes · Work both in senior and skilled nursing homes, taking care of residents · Ability to make use of the Hoyer lift, dressing, undressing and transferring the bed bound · Adhere to infection control and safety procedures which are in accordance with agency policy · Assures all patient information remains private and confidential to ensure the protection of the employer. · Observe patients' conditions, measuring and recording food and liquid intake and output and vital signs, and report changes to professional staff. · Deliver messages, documents · Turn and re-position bedridden patients, alone or with assistance, to prevent bedsores. · Provide patients with help walking, exercising, and moving in and out of bed. · Provide patient care by supplying and emptying urinal, applying dressings and supervising exercise routines. · Prepare, serve, and collect food trays. · Clean rooms and change linens. · Bathe, groom, shave, dress patients up to prepare them for an appointment

· Follow Medication procedure without errors; which includes independently reading and following directions of all medical information through RN/LPN instructions. · Document and maintain accurate data, communication logs, medication administration records (MAR), and all other pertinent information required. · Report changes in resident's condition to manger and/or supervisor. · Successfully complete all scheduled trainings as required. Attend team member’s meetings as assigned. · Document and maintain accurate progress notes and services concerning residents include all data and communication logs. · Assist with daily living activities in the facility. · Observe/monitor individuals/residents and document behavior and incidents as needed. · Follow dietary needs of residents, community activities, or physically assist individuals/residents with daily living skills in one-on-one instruction. · Daily decision-making and the resolution of minor problems; Supervise 4-5 team members on overnight shift. · Actively respond to emergencies and follow company policies and procedures in such cases. · Perform housekeeping duties, such as cleaning, washing clothes and dishes, as well as, running errands as assigned. · Ensure all chemicals are locked and secured · Hourly logs of room checks and resident toileted/changed as assigned
