I work with different clients providing personal assistance with: bathing, dressing, grooming, nail /skin care, back care, foot care, shampoo, oral hygiene, ambulation, transfers, range of motion exercise, safe use of equipment and assistive devices (wheel chair, walker, crutches, cane), change of position, with toileting, commode, incontinent care, meal planning- cooking and nutritional guidance, and assistance with feeding, transportation services, light housekeeping services and laundry, medication reminders, exercise and physical assistance, conversation, companionship and mental stimulation, assists in the access to and documentation of medications that are self-administered by the client, reports any observed or reported changes in the client's condition and/or needs to supervisor, follows the reporting guidelines on the Individualized Service Plan, documents care provided and completes the forms required for the client's record, notifies supervisor of educational needs, maintains confidentiality in all aspects of the job, attends in-service programs to meet, compliance requirements, maintains appropriate communication with the Agency Administrator, Client Care Manager, and Director of Operations (written and verbal), treats clients, families, co-workers and office staff with respect and all other duties as assigned.
