Coordination of services to the elders who are 65 years and up who live in their homes or with others. Purpose of providing services to them is so that they may continue to live at home with support. My job is to find caregivers from contracted vendors who will go to the elders homes and provide services such as personal care, homemaking, meal prep/cooking, meds reminders, medical escort, transportation etc. I visit each elder every 6 months to assess or reassess their continued needs of these services. I also stay in touch with them via telephone calls as needed and interact with their families. I document all home visit and other communication on the computer. I enter authorization for all their services on the computer so the caregivers can get paid. Attend weekly meetings with Interdisciplinary team in a medical clinic where clients medical and social needs are discussed or reviewed.
