• Assisting clients with activities of daily living including but not limited to: bathing, toileting, dressing, cooking, eating, cleaning, errands, attending medical appointments, taking medications • Providing companionship for clients and spouses of clients under care to improve overall well being • Assisting clients with rehab or therapeutic exercises under the supervisions of the nursing or medical director depending on whether client lives at home or in a nursing facility • Writing detailed notes for other aides as well as the nursing or medical director related to patient care • Informing nursing/medical director of changes in client’s health, mental status or overall wellbeing and helping to come up with solutions to address those changes • Answering or providing information to answer any questions the patient’s family may have regarding a client’s care as well as assisting with transitions and being available for clients during transitions like home care to hospice/palliative care • Providing emotional stability for spouses and family members during palliative care transitions as well as recognizing transitions in palliative care and notifying nursing/medical directors and family members of these transitions Please see resume for further
