Nursing Assistant. Emerald Heights, Skills Nursing 02-11-2015 to present. On call. 2017. -Provides the delivery of nursing and nursing-related activities for residents within established guidelines and accordance with federal and state regulations. -Reports all change of resident condition to the licensed nurse and supervisor. -Monitors residents throughout the day without interfering with their privacy. -Provides resident-directed services that honor resident lifestyles and preferences. -Assists residents with daily living activities such as dressing,dining,hygiene, bathing, changing soiled cloths and bedding. -Performs transferring of residents utilizing lift or mechanical lifts in accordance with the resident plans of care. -Keeps residents rooms in a neat and orderly manner while honoring resident preferences. Creates an environment, with neighborhood team. -Monitors residents with mobility issues. Practices established preventative skin care techniques. -Act as emergency responder with AED and emergency supplies for community wide emergencies.

- Observes, documents, and reports changes in patient activity and/or symptoms such as vital signs, respiration, discomfort, intake, output, weight, and bleeding. - Assists with or prepares patient for transport including moving patient to and from bed to gurney or wheelchair. Transports patients, patient belongings and valuables, blood products, supplies, pharmaceuticals, equipment and specimens to and from various locations. - Acknowledges patient inquiries; responds or refers inquiries appropriately. Answers patient call lights; determine nature of problem; respond within scope and/or obtain appropriate resources. -Assists RNs and other healthcare providers with wound care and patient procedures as directed. - Accountable for following the guidelines for attendance, punctuality and overall dependability. Accountable for effective performance and follow-through of all assigned responsibilities and for completing responsibilities within designated (or agreed upon) timeframes. Completes responsibilities in a manner consistent with organizational policy, goals and values. - Responsible for the "Accountabilities" associated with this position in support of the organization. -Performs other duties as assigned for the health care team.

Care for up to five different clients on a rotating schedule. Assist client with daily living activities. Personal, meal preparation, grooming, housekeeping. Medications remaining for clients who required. Drive client to all appointments, pick up groceries and collect medications. Help patients to complete prescribed therapy and exercise routines. Kept client cares neat, clean and free of safety hazards.
