Client Needs: Schedule: Current Month: 2-hour visits as needed. Beginning Next Month: 2-hour visits once weekly. Days/Times: Flexible, Monday–Sunday (AM/PM) Services Needed (ADLs): Bathing Grooming Toileting Transferring & Mobility Caregiver Responsibilities: Assist the client with bathing, grooming, and toileting. Provide safe assistance with transfers and mobility. Follow the client's preferred schedule and care routine. Promote the client's safety, comfort, and independence while providing compassionate care. Notes: Approved services are limited to the listed ADLs only. Schedule is flexible and will transition to 2-hour weekly visits starting next month.
