Member lives alone in apartment, works, and is normally independent with ADLs. PMH includes: Chronic Back Pain, Cerebral Palsy (WC bound), h/o Quadriplegia, but able to stand, able to self-transfer+ from bed to WC, Hypothyroidism, GERD, Lymphedema, Dep, ILD, Osteoporosis. Member fell while tx from WC, resulting in Lumbar fracture. Member states she needs assistance getting in/out of bed and bathing. address 10536 Southeast 85th Avenue, Happy Valley, OR. Special needs: Lifting legs across to get into the bed, bathing, and light housekeeping (floors) 1st Visit: Evenings (5 PM - 9 PM) Monday through Sunday Care schedule: 3 hours per shift Care Plan being used?
