We are seeking a compassionate and reliable Home Health Care Worker to provide daily living assistance and personal care for an individual living with Multiple Sclerosis (MS) who uses a wheelchair. The ideal candidate will be patient, dependable, and have experience with mobility assistance and home-based caregiving. Key Responsibilities: • Morning Routine & Mobility Assistance: o Assist with getting the patient out of bed safely each morning o Transfer patient to and from wheelchair using proper techniques and/or transfer aids o Provide assistance with toileting as needed • Personal Care: o Help with showering and hygiene routines o Assist with dressing and grooming, including hair, dental care, and skincare o Monitor for and report any changes in the patient’s physical or emotional condition • Meal Support: o Prepare nutritious meals according to dietary needs and preferences o Assist with feeding if necessary • Light Housekeeping: o Perform light cleaning tasks related to the patient’s care and comfort, including laundry, changing bed linens, and maintaining a clean and safe environment • Companionship & Emotional Support: o Engage in conversation and companionship o Support the patient’s mental and emotional well-being Qualifications: • Prior experience as a caregiver or home health aide, preferably with neurological conditions or mobility-limited clients • Ability to perform safe transfers and mobility assistance • Strong sense of empathy, patience, and professionalism • Basic cooking skills and ability to follow simple dietary instructions • Reliable transportation and punctuality • Certification in First Aid/CPR preferred • [Optional] CNA or HHA certification a plus Physical Requirements: • Must be able to lift and assist with transfers (minimum of 50 lbs) • Ability to perform tasks involving bending, kneeling, standing, and reaching Automatic wheelchair, sit-to-stand equipment and elevator available in the home.
