The assessment you complete will provide essential information back to the Care Manager within the Solutions for Caregivers team. You are the "eyes and ears" of the Care Manager while you are in the member's home. You are using the assessment to gather information, document your observations in the home, make recommendations, and evaluate the safety of the care recipient. Your complete assessment will provide the Solutions for Caregivers team insight to better meet the needs of the care recipient and the caregiver in the home, if present. To qualify, you need to be a currently licensed RN or FCA with a blood pressure monitor and stethoscope. *MUST BE COVID VACCINATED*
