I was employed as the head Social Services Director of a 120 bed Nursing Home facility and am a licensed registered BSW Social Worker. My duties were as follows, but not limited to such. I was encharge of the facility admissions/discharges with assistance given to the resident an their families in a effort to make sure home care needs were going to be met prior to discharge. I assisted the facility Activities Director with various resident daily events, provided classes to staff, residents and resident families. Attended resident care conferences on a routine basis, documented resident progress in their medical charts and actively involved in making sure that each residents psychology and social needs were met. Often helped resident families with filling out paperwork for persons applying for medicaid, power of attorney and guardian conservatorship as needed. The company also assigned me to be a trouble shooter sending me to other facilities to assist other Social Workers upon hiring or post hiring as needed. I also was involved with many community programs for the elderly such as Council on Aging, Senior Center, Community Mental Health Services an numerous others.
