He was a 68-74 y.o. Vetran suffering from Sundowner Syndrome, PTSD, Congestive Heart Failure, Cardiomyopathy, Atrial Fibrillation, Tachycardia and High Blood Pressure. This put him at an extremely high risk of Stroke. He was also suffering from Severe Muscle Atrophy. My duties included: * Assisting or fully moving him from his bed to the wheelchair then to his recliner to watch television in the living room * Assisted him with his daily showering, grooming and checking for bed sores/pressure ulcers * Handled most of the grocery shopping * Cooked breakfast, lunch, snacks and dinner * Ensured all meals and snacks I prepared were heart healthy as well as reduced sodium * Monitored fluid intake and output for CHF * Medication Management * Checked Blood Pressure 3 times daily (his BP would tend to run low) When this occurred I would elevate his feet above his heart, have him pump his feet, give additional fluids, call 911 and immediately notify his daughter * Transportation to all standard doctor appointments at VA or other offices * Worked with him doing Physical Therapy using one ore more devices: Hoyer Lifts, Gait Belt, transfer board, shower bench, shower chair, commode, hand weights, resistance bands and various balance balls * Repositioned him initially every 4 hours with pillows when he was bedridden * Provided continuous companionship * Created an evening routine that helped to REALLY ease his Sundowner Syndrom

Provided Post-Surgical care for a 63 y.o. female who was having her third major back surgery in under 2 years. She was essentially bed bound, only able to get up to use the restroom on rare occasion, following a near 2.5 month recovery period, post-surgically, in an adjustable bed. I was able to work with Sheri on her Physical Therapy, after this period and within 45 days she could move around her home for 10-25 minutes at a time with use of a walker in front of her, gait belt around her upper torso and me behind her holding the belt and pushing a wheelchair. My additional duties included: * Medication Management * Daily sponge bathing, changing diapers, emptying/sanitizing her catheter collection container and tubing, changing her Pure-Wick, linen and pad changing and checking/changing the dressing on her incision along her back. * Checking for pressure sores, and notating any irritation or redness in daily chart. * Rotating her every 4 hours to prevent sores and help her comfort level. * Grocery shopping, running miscellaneous other errands. * Preparing breakfast, lunch, snacks and dinner. * Light housekeeping (vacuuming, dusting, sweeping, mopping and laundry) * Feeding, cleaning up after and playing with her 3 dogs daily, as well as taking them to the groomed twice a month.

Provided 24 hour Live-In Care for a 67-77 y.o. female patient with Type 2 Insulin Dependent Diabetes, Congestive Heart Failure, End-Stage Renal Failure and Alzheimer's. My duties included: * Transportation to and from Dialysis 3-4 times each week * Extensive Daily Charting * Transferred 100% of her weight. * Assisted or fully performed daily bathing, grooming and checking for bed sores/pressure ulcers * Made breakfast, lunch or snack (depending on her appetite) and dinner * Ensured all meals I prepared were heart healthy, reduced sodium and in accordance with "My Plate" guidelines * Monitored fluid intake and output * Medication Management * Checked Blood Sugar 6 times daily (before and after all meals) * Administered all Insulin doses according to Blood Sugar results * Took Blood Pressure 3 times daily, however as often as 5 times a day, as she later became hypotensive * Treated her severe cellulitis * Worked with her doing Physical Therapy on average 3 times a week * Went with her to her weekly hair appointments * Companion care was a huge part of my time spent with Lucille. Whether it was sharing meals with her, watching her favorite older movies, engaging her in conversations that always cheered her up, playing her Elvis records, making her laugh, or simply holding her hand and calming her down anytime she became scared, anxious or confused.
