About Kristi

I have 18 years of experience caring for adults in both inpatient and outpatient settings. I enjoy working with seniors and currently volunteer weekly at the Lake Oswego Adult Community Center, working with people with mild cognitive impairments and dementia. I worked at OHSU for over 12 years as a Social Work Specialist where I provided both patients and families support and education around community resources, systems of care, legal systems and insurance needs. I found that by connecting people to a needed resource , it can help them remain active and independent. I look forward to bringing that rewarding aspect to my role in care giving. In my spare time, I enjoy cooking, exercise ( I truly believe that a walk and some fresh air can transform your day!) and spending time with my husband and two sons.

I am currently a volunteer with a day program withing the community center called "Our Place at the Center" which is a day program for seniors with mild cognitive impairment as well as other kinds of dementia such as Alzheimer's. Duties include leading participants through a structured day that includes meals, art , music, movement and cognitive stimulation.

Inpatient psychiatric care discharge planner. Duties included making appropriate follow up for post hospital care that included mental health as well as medical care appts. This often included setting up home health care and arranging for durable medical equipment. I also arranged discharge to various facilities including skilled nursing as well as sub-acute care. I facilitated family meetings where I provided support as well as education around community resources, systems of care, legal systems, insurance and financial information. Other duties included insurance verification, back up utilization review, maintaining state contracts for committed patients, and improving community relationships with clinics to ensure I stayed on top of the ever-changing atmosphere of outpatient care.

Identified and assisted clients in finding appropriate housing as well as other resources such as food, clothing, furniture, primary health care and transportation.
