The Quality Coordinator identifies opportunities for performance improvement. The quality coordinator is responsible for collecting and/or coordinating the collection of quality and compliance-related data, which includes but is not limited to the review of patient records and/or other clinically-related documents and logs. The quality coordinator identifies deviations in generally accepted performance standards, recognizes patterns and trends, and reports findings to the appropriate leadership. The quality coordinator provides education, orientation, and instruction/competency-assessment to designated staff. As a member of the organizational's quality department, the quality coordinator takes lead responsibility on assigned company-wide processes or committees.

* Assisted the Administrator in the planning, organizing, and directing of patient care services in accordance with current standards and regulations so that the highest degree of quality care was maintained at all times. * Directed patient assignments, staff scheduling, coordination of referrals, patient volume distributions, and assisted in meeting financial targets by effectively managing utilization, productivity, personnel and supplies. * Ensured adherence to clinical best practices to promote optimal patient outcomes and patient experience and ensured that patient needs are continually assessed through development, implementation, and updates to the individualized plan of care coordination with the Case Manager. * Participated in team meetings and represented the agency to the community. * Responsible for the oversight of a clinical multi-disciplinary team of Registered Nurses, Licensed Vocational Nurses, Physical Therapists, Medical Social Workers, Chaplains, and Home Health Aides, and managed an active patient census of approximately 100 patients. * Supervised, supported, and educated staff at the point of service in the field. * Supported an efficient and accurate revenue cycle by enhancing capture of appropriate revenue and managed the flow of documentation within the department. * Provided direct patient care at times of high census and/or for emergent need and took on other managerial tasks and projects as assigned by the Administrator.

* Under the supervision of the Director of Nursing, responsible for the overall management of approximately 20-30 Home Health, Hospice, and Private Duty patients. * Planned, organized, and directed all patient care services for patients in the designated caseload in accordance with current standards and regulations, and ensured the optimal degree of quality care was maintained. * Responsible for coordinating the interdisciplinary team to implement the established plan of patient care and ensured effective management of visit utilization and control of expenses. * Assisted in agency wide orientation, competency evaluations and consultation to clinical staff related to specific expertise as requested by the Director of Nursing.
