Completed physical checks:(1:1, line of sight), monitor, documented or otherwise reported observations of patient unusual and harmful behavior. Restrained or aid patients as necessary to prevent injury and maintained patient's restriction to assigned areas. Admits, transfer and discharge patients; transports patients; collect and take specimen to lab; assist with special procedures; telephone answering; document and assist in Unit record-keeping. Make beds; clean and care for equipment; carry out isolation precautions; practice medical asepsis and infection control; practice standard precautions; observe oxygen precautions. Protect patient privacy and maintain confidentiality; keep call signals within patients reach; answer call signal promptly. Bathe patients; assist with oral hygiene; hair care and shave patients; assist with dressing and undressing. Turn and position patients; provide range of motion exercises; transfer patients to wheelchair/stretcher; assist with ambulation. Check and record vital signs; measure height and weight; measure intake and output; observe patient response to care; report and records observations of patient's conditions. Transport patients to and from dialysis; assist with set up; clean dialysis equipment and store supplies properly; clean common areas.
