I took care of caseloads that came from my internship at Kindred at Home hospice (PeopleFirst Hospice). I went to the patients' homes or long-term care facilities on a schedule that was in line with the volunteer plan of care to help them. This is what I did: I told the Social Worker and/or Volunteer Coordinator about changes in the patient's condition. I also made sure to write down that these changes or concerns had been told about and to whom. I sent the completed Volunteer Companion Visit form to the Volunteer Coordinator within one week of each patient visit, following the agency's rules. I also followed the rules of the organization and worked with other people on the interdisciplinary team. I kept all confidential and protected health information safe and only used and shared what I needed to do my job.* Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs. * Monitor, evaluate, and record client progress according to measurable goals described in treatment and care plan. * Maintain records of patient care, condition, progress, or problems to report and discuss observations to Social Worker and/or Volunteer Coordinator. * Provided patients and families with comfort, emotional support and instruction on preparing for the approaching death. * Managed a caseload of more than 5 clients at any given time.
