Assist incoming youth who are victims of child abuse, neglect and sexual abuse adjust to group home living by discussing the expectations and behaviors deemed appropriate. Initiate the assessment process for all incoming residents to include triage, gathering of demographic and historical information to assess for any crisis situations and develop plans for ongoing safety. Ensure all youths educational goals, needs and welfare are being met by IEP's, ISP's, tribunals, disciplinary meetings, administrative proceedings, conferences, etc. Review progress notes from therapists, counselors, mental health facilities, and the psychiatrist to ensure adequate services are being provided. Coordination of monthly reports, discharges, Individual Service Plans, and assessments. Observe resident behavior and discuss concerns with the resident in an effort to achieve improvement. Provide group training to clients in implementing behavior modification techniques identified in treatment plan. Update treatment plan goals, recommendation for continued services, areas needing improvement and goals met every 6 months.
