Primarily Hospice Caregiver Served three clients who are either hospice, Dementia and Alzheimer's disease and with disabilities either at clients home, eskaton or brookdale facility. Provided the following assistance: Personal care includes bathing, oral hygiene, dressing, removal of facial hair, nail care, skin care, back care. Mobility care includes ambulating, transferring, lifting, positioning, or assisting clients to and from beds, chairs, wheelchairs, toilets, tubs and showers. Performs basic procedures such as vital signs, weights, recording bowel activity emptying colostomy bags and foley bags, treatments and application of lotions. Transports, ambulates, and transfers residents using wheelchairs, walkers, hoyer lifts, and other devices. Assist in preparing meals, cooking, cleaning, shopping, budgeting, and medicine administration. Companionship with social activities and exercise – range of motion. Trained and assisted clients with Alzheimer and Dementia.

Serving a 78 year old female client who is legally blind with limited mobility, HBP and Diabetic. Provided the following assistance: Personal care includes bathing, oral hygiene, dressing, removal of facial hair, nail care, skin care, back care. Treatments and application of lotions. Transports, ambulates, and transfers residents using walker and other devices. Assist in preparing meals, cooking, cleaning, shopping, budgeting, and medicine administration. Companionship with social activities and exercise – range of motion. Trained and assisted clients with Alzheimer and Dementia.

Provided care for 77 year old male, hospice, dementia, Alzheimer's, diabetic, pacemaker, hearing loss,, and HBP clients). Assisted in checking the pulse rates, temperatures, and respiration rates of clients and helping them take their prescribed medicine. Assisted in personal hygiene care such as help with bathing, grooming, shaving, hair washing, hair brushing, dressing and skin care as well as in repositioning to avoid bed sores, adult briefs, sponge baths and transfer client frequently. In addition, breathing treatment, catheter change, canula/ track change, and operating wheel chair. Assisted in domestic tasks such as making the bed, doing laundry, preparing special meals, light housework (i.e. vacuuming, dusting, sweeping, washing dishes, kitchen maintenance), shopping for groceries, errands to pay the clients bills. Traveled with clients to doctor’s appts. Provided Companion care to help client stay socially engaged at home and also continuing to visit friends, family and other social events. For example, social activities included board games, gardening, reminiscing, other interest client may have. Provided safety and supervision due to client has history of wandering. Provided record keeping such as documenting client’s physical problems as well as medical and emergency information of clients past and current history.

Provided care for disabled, chronically ill, and cognitively impaired clients (quadriplegic female, diabetic senior citizen, and HBP clients). Provided safety and supervision of client’s problems or needs by maintaining a safe home environment in compliance with IHSS.
