Assist department public health professionals in providing public health services to individuals, families and communities including local schools and institutions. Assess clients and or family health needs for case staffing with public health professionals. Assessment include measurements of vital signs, height, weight and assessment of social, cultural and environment needs and assets. Provide health education/promotion interventions to individuals, families, or groups in home, at local schools, senior centers, or in community settings using developed curricula and programs under directions of PHN. Deliver prescription drugs and patient care supplies as instructed by PHN, instruct patient/families in proper use of drug or supplies and discuss possible side effects as directed by PHN. Conduct patient visits in home or at community facilities to meet program guidelines, including assessment of health needs, monitoring of health status, appropriate documentation, and feedback to the public health professional on findings outside defined parameters. Evaluate and re-asses client needs. Conduct home visits alone with PHN and or/other health care professional, explain illness, treatment and care being given. May interpret/translate for health care provider and patient of health problems, needs, responses of patient and concepts of culture beliefs and practices.
