(ADLs) Assist with bathing, grooming, and dressing Help patients with toileting and use of bedpans or commodes Support with feeding and ensuring proper nutrition and hydration Help with ambulation and repositioning patients in bed or wheelchairs Perform basic wound care as directed Health monitoring and reporting Take and record vital signs, including temperature, blood pressure, and pulse Monitor and document patient intake and output Observe patients for any changes in health, behavior, or condition and report them to nurses Environment and safety Answer call lights and respond to patient needs promptly Keep patient rooms clean and sanitized Change bed linens and restock supplies Ensure patient safety and assist with transfers and lifting Other responsibilities Document care activities and provide reports Provide emotional support to patients and their families
