Support and promotes the Sanctuary Model and its Commitments. Creates and manages reentry client files Provides case management services by coordinating with other agencies, resources, and professionals within a planned framework of action toward the achievement of established goals Identifies community-based resources and information needed to accommodate the needs of Office of Reentry program participants and their households, and make referrals when appropriate Seeks to promote healthy relationships between clients, their families, and others involved in their successful reentry process Organizes and documents all client services & program referrals to be routinely entered into the client reentry database system Participates in the management of service coordination meetings to review, report, and monitor client progress Assists in preparing classes, group workshops, special programs, and supportive services Responds to crisis situations and uses appropriate crisis intervention techniques Assists in onsite & offsite event monitoring and supervision of Office of Reentry clients Assists in execution of the needs screenings, assessments, and Community and Resource Engagement (CaRE) plans during the intake process Maintain and collect meaningful client data (i.e., Key performance indicators) to produce statistical reports and measurable outcomes Attends regularly scheduled and assigned meetings & training
