My skill level and experience is working with dementia and Parkinson’s patients in addition to a variety of other health care needs. I am proactive, professional, compassionate, devoted, dependable and committed to excellence. I have a natural gift and a heart for the elderly and it is my way of giving back. My professional experiences are as follows: • Provide personal care including: bathing, grooming and oral hygiene. • Assist client with dressing, toileting activities and feeding. • Maintain daily records of vital signs. • Support good skin care to prevent skin breakdown by turning and repositioning the client and actively encouraging movement. • Assist client with active and passive range of motions as defined by the plan of care. • Assist client with walking, transferring as needed. • Assist with doctor visits, scheduling appointments and transportation. • Observe client’s physical, mental emotional conditions and provides report of changes to the nursing supervisor. • Document intake and output.
