-Monitor patient condition by observing physical and mental condition, intake and output, and exercise. -Support patients by providing housekeeping and laundry services; shopping for food and other household requirements; preparing and serving meals and snacks; running errands. -Assist patients by providing personal services, such as, bathing, dressing, and grooming. -Help patients care for themselves by teaching use of cane or walker, special utensils to eat, special techniques and equipment for personal hygiene. -Help family members care for the patient by teaching appropriate ways to lift, turn, and re-position the patient; advising on nutrition, cleanliness, and housekeeping. -Record patient information by making entries in the patient journal; notifying nursing supervisor of changing or unusual conditions. -Maintain a safe, secure, and healthy patient environment by following asepses standards and procedures; maintaining security precautions; following prescribed dietary requirements and nutrition standards; Update job knowledge by participating in opportunities. -Protect the home care agency by adhering to professional standards, home care policies and procedures, federal, state, and local requirements. -Enhance service reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments.

Performs and assists with basic patient care activities of daily living (ADL’s) within unit specific defined limits and job scope, such as assistance with patient elimination, hygiene, comfort, safety, nutrition, and progress activity.(EF) 2. Performs and documents, where applicable, care administered, diagnostic measurements, treatments and procedures as determined by skills training and validation in accordance with established policies and procedures. Reports observations, conditions and problems of patients to licensed personnel to achieve desired patient outcomes.(EF) 3. Performs and documents, where applicable, accurate vital signs. (EF) 4. Responds to the Nurse Call System, contacting nursing personnel as appropriate, and follows through with meeting patient needs. (EF) 5. Assists with maintaining and cleaning of equipment and supplies as established on assigned unit. Assists with positioning and transporting of patients. (EF) 6. Seeks guidance and validation of knowledge base, skill level and decision making as necessary, especially in areas of question, from preceptor and assigned licensed personnel.(EF)
