Nationwide partnership combines Lacuna Health’s proven registered nurse-led care engagement solutions with CareLinx’s nationwide, tech-enabled caregiver marketplace.

Lacuna Health, a leading nationwide provider of registered nurse-led care engagement solutions, and CareLinx, a leading nationwide, tech-enabled caregiver marketplace, have announced a strategic partnership to improve access to affordable, coordinated, quality home care for families, large health systems, and insurance companies.

The Centers for Medicare & Medicaid Services (“CMS”) recently announced a final rule allowing non-skilled in-home care services as a supplemental benefit for Medicare Advantage (“MA”) plans in 2019. The Lacuna Health and CareLinx partnership combines core capabilities of each company and will help address the growing demand in the marketplace by creating a differentiated experience for users.

Lacuna Health was established in January 2018 as a wholly owned subsidiary of Kindred Healthcare, LLC and continues to rapidly evolve from its early beginnings as Kindred’s Contact Center. With over 70 highly trained registered nurses and licensed practical nurses available 24/7 across all 50 states, Lacuna Health will provide care engagement services to over 500,000 patients and caregivers in settings including hospitals, home healthcare, skilled nursing, and physician groups.

CareLinx professional caregivers leverage proprietary digital care plans to better assist the elderly, individuals with disabilities, and people with chronic health conditions with activities of daily living. CareLinx seeks to improve quality of care delivery for these patients while reducing the overutilization of in-patient healthcare costs for families through in-home care solutions.

“Patients and families are often ill prepared and blindsided when thrust into a role of managing the care for a loved one. We are committed to expanding access to high-quality resources and caregivers for patients and families who are navigating the healthcare system from home. As the industry continues to evolve, we believe this will become increasingly important to the communities we serve,” said Dr. Brian Holzer, Chief Executive Officer of Lacuna Health.

Building on over seven years of operating success, CareLinx now boasts a nationwide network of over 300,000 tech-enabled caregivers who provide personal in-home care for families and support safe transitions of care, which is critical to the success of large health systems and insurance companies. The CareLinx platform revolutionizes transparency and facilitates active coordination in the post-acute care environment, leading to increased quality of care, better health outcomes, and significantly reduced total cost of care.

As part of this partnership, Lacuna Health will begin offering its customers screening for caregiver services as patients transition home from hospitals or other post-acute settings. Similarly, CareLinx will begin offering Lacuna Health’s proven care engagement solutions to provider organizations and insurance companies to create a fully integrated caregiver and care management platform.

“Our partnership with Lacuna Health is poised to be a game changer for the healthcare industry. With our successful integration of Lacuna’s skilled care management into our nationwide tech-enabled home care platform, we’re offering healthcare clients who haven’t built their own in-house care management organization a complete outsourced solution. Helping healthcare organizations efficiently extend more care into the home and community based setting through a comprehensive Care Management plus Tech-Enabled Home Care platform is a real differentiator,” said Sherwin Sheik, Chief Executive Officer of CareLinx.
